The Hall Of Fame is an innovative Quality of Life Program developed by Signature HealthCARE designed to honor the lives and accomplishments of our residents, stakeholders, volunteers, and others who have had such an impact on our centers, communities, and nation.
On October 9, 2014, 87 Signature HealthCARE centers across the southeast and midwest will induct over 350 distinguished individuals into the Hall of Fame, in local ceremonies featuring celebrity speakers and presentations about the inductees lives and accomplishments.

Signature HealthCARE (SHC) of North Florida has received accreditation for 2014 as a skilled nursing facility actively performing Quality Assurance and Performance Improvement activities.
The accreditation, presented by independent accreditor Providigm, LLC, demonstrates that the facility is continually assessing residents’ quality of life with regard to concerns such as pain, dignity, respect for resident choice, and quality of care problems such as whether there are enough staff to meet resident needs, weight loss, infections, rehabilitation following acute injury or illness, and prevention of readmissions to hospital.

Following five months of hospitalization for multiple co-morbidities that resulted in acute respiratory distress, Wanda Bullig arrived at Signature HealthCARE of North Florida for physical rehabilitation.
She came to us with a medically complex history including respiratory failure, congestive heart failure, and complications from a mitral valve replacement, but was resolved not to let it hold her back!

Everyone dreams of a couple of days on the beach. The sunshine, sand, waves, and laid back atmosphere create an environment of relaxation and peace. Elders from five Florida Hometown homes spent two days on the beautiful beaches of Cape San Blas, Florida and they burst that environment of relaxation and peace wide open with the first Amazing Race Retreat.
Elders from The Bridge at Bay St Joe, Signature HealthCARE of North Florida, Signature HealthCARE of Courtyard, Chautauqua Rehabilitation and Nursing Center, and Washington Rehabilitation and Nursing Center came together at William Rish Park on Cape San Blas and played a two day game of Amazing Race. They created team flags with points awarded for time completed and bonus points for creativity, they built sandcastles for points, they had to hunt down puzzle pieces and put their puzzle together for timed points, and their last event was creating a platform only out of provided materials to transport their team mascot to the water’s edge and back. During the two day event, each team had their created flag and mascot to protect. If either the flag or their mascot were left unattended, another team could kidnap them for ransom points. The team whose flag or mascot was kidnapped had to draw a ransom note and pay the required points to the kidnapping team.

Signature HealthCARE (SHC) of North Florida is proud to announce that it has received a five-star survey rating from the Florida Agency of Health Care Administration, the state government agency tasked with oversight of nursing home quality.
Signature Healthcare of North Florida is a 180-bed nursing community located at 1083 Sanders Ave., in Graceville, Fla. It is one of 72 nursing facilities operated by Louisville, Ky.-based Signature HealthCARE LLC.
Under the five-star rating system, nursing homes are assessed in order to qualify to participate in the Medicare & Medicaid program. The requirements under this system serve as a basis for survey activities for the purpose of determining whether a facility meets the requirements for participation in the CMS program.

I have been employed here for almost 10 years. I live here in Graceville and have for 35 years. My uncle is the fire chief here; his name is Tommy Dennis, and he has a daughter named Tonya. Tonya, her sister Christy and I grew up like sisters. We are within a year of age, me being the oldest.
My cousin Tonya has two boys and is a single mother. She has had a rough time the past few years and is diabetic. Her partner died two years ago of cancer, and they had no medical insurance at the time. Tonya was a truck driver and was able to get a job driving a transportation bus for the elderly, but she still did not have insurance.
